Your Visit
Our initial appointment will begin with a thorough medical and dental history in addition to an oral exam. We want to understand your previous dental experiences and give you the opportunity to share your thoughts and concerns with us. In our opinion, an informed and involved patient is more likely to achieve a higher level of healthcare.
The oral exam includes a hard tissue (teeth) and soft tissue (gums) examination as well as an important screen for oral cancer. A preliminary consultation explaining your diagnosis and treatment options will be discussed at the end of your first visit.
Occasionally, treatment can be performed the same day as the consultation. However, a complex medical history or treatment plan will require an evaluation and a second appointment to provide treatment on a subsequent day.
X-Rays (if applicable): You can either bring them with you or ask your previous doctor to send them to us before your appointment date. Digital films are preferable and can be emailed to needhamperio@gmail.com.
List of current prescription medications you are taking.
List of any medical conditions.
Filled out patient registration forms: Health History, Consent Form, Office Policies, Privacy Policy. Bringing these four forms to your appointment will help us expedite checking you in.
Insurance information: If you have dental insurance, please bring your card and any necessary completed forms, including referral paperwork (if applicable). This will also save time and allow us to help you process any claims.
